I didn't find Fear that scarey - it's more creepy, but generally there aren't any negative effects. And the series has moved away from horror towards shooter over the 3 games (they're all good though).
I find fear or scare factor in games is proportional to how much you stand to lose. Dead Space 2 on hardcore (where you can only save 3 times throughout the entire campaign) was properly scary. Especially if you were low on ammo and/or coming up to a bit you knew gave you trouble...
Demon's and Dark Souls have made me literally jump, but lose most of the scare factor when you know the levels. It can make you jump when something comes up behind you while you're occupied in front
